Caring for you in your sunset years
As we age, many of us develop multiple, complex health needs. At Crossways Health Centre we enjoy caring for the older generation, and are prepared to spend the time required to care for our patients appropriately.
If you’re over the age of 75, we encourage you to have a 75+ year old Health Assessment. These are usually done once every year or two. At the start of the assessment you’ll be visited by our Enrolled Nurse, Joyce, who is experienced in rehabilitation. She will come to your home and check for any trip hazards such as steps or mats, assess the need for handrails around your home, and check your overall physical and emotional health including social needs.
You’ll then come to the clinic for an appointment with your doctor who will finish the assessment and complete the general check of your overall health. These assessments are bulk billed.
Our doctors can carry out drivers licence medicals for anyone who needs one, and encourage you to stay fit and active for as long as possible. If you’re taking multiple medications we can arrange a pharmacy Home Medication Review to make sure complications are avoided wherever possible, and that you understand what all your medications are for.
We work with your relatives and local support services to keep you in your own home wherever practical. If you’re thinking about a transition to different accommodation, we can help with the process. Although you may not want to think about it, we do encourage you to consider things like Advance Care Directives, Enduring Power of Attorney and Enduring Guardianship.
Dr Earp and Dr Henderson share the care at three local nursing homes, generally visiting weekly with the help of our Registrar. Each month they swap over care responsibility so they remain familiar with all the residents.
